Overview
On Site Podiatry delivers podiatry services to Aged Care Facilities, Group Homes, Medical Centres, Corporate clients and Home Visit clients. We are committed to maintaining the privacy of personal information in accordance with the Australian Privacy Principles.
Collecting Information
As our client, we may collect some or all the following information about you that is reasonably necessary for the purposes of providing our services to you. This may include, but is not limited to:
- Name
- Address
- Date of Birth
- Phone number and email address
- Next of Kin contact details
- Medicare and DVA numbers
- Health fund details
- Health information and medical records including digital, paper, photo, video, diagnostic and laboratory reports created by us, or provided to us by other health care providers involved in your care.
- Additional solicited information provided to us by the individual
- Unsolicited personal information that is reasonably necessary for providing our services to you will be retained. Any unnecessary unsolicited personal information we receive will be destroyed unless it is unlawful to do so.
For clients in Aged Care Facilities and Group Homes, your medical assessment, care plans and treatment progress notes are kept on site, within the facility. For all other clients, these are kept by us in a secure location.
We retain copies of referrals and correspondence from other parties involved in your health care such as your GP. All records may be held on secure premises in hard copy format or scanned or typed into a secured Electronic Medical Records system, or both.
Use and Disclosure of Personal Information
We only collect personal information that is directly necessary for providing our services to you. We only disclose your information to those other parties that you would reasonably expect us to, such as your General Practitioner or other service providers involved in your health care, unless you have explicitly permitted us to do so, or if we are required by law or for safety reasons, or any other circumstances where permitted under the Privacy Act.
We do not use or disclose your personal information for direct marketing purposes. We also do not disclose your information to any recipient outside Australia. All reasonable steps to will be taken to ensure that the data we collect, use or disclose is accurate, complete and up to date.
Access to Personal Information and Complaints Procedures
Our clients can access their personal records on written request to our Privacy Officer.

Complaints about a breach of the Australian Privacy Principles must addressed to the Privacy Officer and be received in writing.
Breaches of privacy are taken very seriously and will be responded to as a matter of priority.
Availability and Review of Privacy Policy
This privacy policy is freely available upon request. This policy may also be viewed on our website: http://www.onsitepodiatry.com.au. This policy will be reviewed from time to time and any amendments will be incorporated into the updated policy and posted on our website.
